Click "Forgot Password?" on the login screen. Then enter the Username associated with your account.
If you also forgot your Username, you will be able to retrieve it by clicking the "Forgot Username?" link at the bottom of that screen.
After entering the Captcha and clicking "Reset my Password", you will receive an email allowing you to reset your password.
